About Us

 

I have provided physical examinations to executives, business leaders and companies for the past nine years.  I was previously the medical director of the executive medical program at Northwestern and developed the protocols and guidelines that were followed in that highly regarded program at that time.


I was born and raised in Chicago.  I graduated from the University of Illinois in 1980 and from Northwestern University Medical School in 1984.  I completed my Internal Medicine residency in 1987 and I have been certified by the American Board of Internal Medicine since 1987.


I have been a member of the teaching faculty at Northwestern University Medical School since 1988 and have been affiliated with Advocate/Illinois Masonic Hospital for several years.  I have been a staff physician at Northwestern Memorial Hospital since 1990 and was previously on staff at Ingalls Memorial Hospital, Palos Community Hospital and Evanston Hospital.


I am certified to perform all classes of FAA medical examinations.  I also perform ATC examinations.  I am an independent HIMS sponsor.

What to bring for an faa examination:

AIRMAN EXAMINATION

  

1. Completed Medxpress confirmation number OR completed printout

2. Any corrective lenses for reading or distance.

3. Any recent FAA medical communications.

4. Any SI or SODA letters.

5. Any relevant medical documentation.

6. A government issued photo ID.

FOR HIMS EXAMINATIONS (IN ADDITION TO THE ABOVE)

 

1. All FAA communications.

2. All documentation related to substance abuse OR

3. All documentation related to SSRI use OR

4. All documentation related to a psychiatric diagnosis or  treatment.

5. Results from all testing that has been previously performed.

6. Any relevant legal documentation.

7. Please request the Blue Ribbon File from the FAA.

ATC EXAMINATIONS

ATC examinations must be authorized through the FAA prior to scheduling an examination.

Can the examinations be charged to my insurance company?


  • The examination is payable by check, cash, credit card or through an agreement with your employer. 
  • ALSO ACCEPTING VENMO AND QUICKPAY

What are the charges for services?

  • Class 1-$120.00.                        
  • Class 2-100.00.                           
  • Class 3-$90.00.    
  • ATC-scheduled with the FAA.
  • HIMS examinations-please call.
  • ATC examinations-coordinated through the FAA.
  • Physical examinations-$210.00
  • Office visits-based upon time spent at a baseline rate of $210.00/hour.
  • EKG-$40.00.
  • Audiogram-$30.00. 
  • Laboratory testing-can be billed directly at a significantly reduced cost or billed through insurance.     

What are the charges for Medical Examinations:

  • Physical examination:  $210.00
  • Office visits:                 Based upon the time spent during the visit.
  • Lab testing:                 Can be billed through insurance or directly paid.
  • Testing can be performed at any certified location, including any of the Quest Diagnostic facilities throughout Chicago and the suburbs.
